Refractory Angina: When the Nitrate Isn't an Option
A single patient, symptomatic on two anti-anginal agents already at target dose. The disagreement isn't about which add-on works best on paper — it's about a six-year-old prescription, for something else entirely, that has quietly taken the guideline-preferred answer off the table.
R.G., a 58-year-old man, has spent most of his adult life on his feet at a machine shop, running lathes and mills through ten-hour shifts he's kept up since his twenties. Two years ago a positive stress test led to a diagnostic catheterization and a drug-eluting stent to his proximal LAD; he was back on the shop floor within six weeks and, until recently, considered that episode closed. His hypertension has been controlled on metoprolol succinate and amlodipine for most of that time, and a high-intensity statin has kept his LDL at goal. What's brought him back is more ordinary and more frustrating than another blockage: two or three times a week now, hauling a die across the shop floor or climbing the two flights to the break room leaves him with the same centered chest pressure that sent him to the emergency room originally — gone within a few minutes of stopping, but recurring often enough that he's started timing his tasks around it.
His workup confirms what the history already suggests: exertional angina recurring despite two agents that should be controlling it, not a new coronary event, and the standard next step from here is well established — add a long-acting nitrate, cheap, familiar, and the best-studied third agent for exactly this problem. What complicates it is a medication that has nothing to do with his heart on paper. He has taken tadalafil 5 mg daily for six years, prescribed originally for a combination of erectile dysfunction and benign prostatic hyperplasia symptoms, and he takes it every day, not situationally. Long-acting nitrates and PDE5 inhibitors both converge on the same nitric oxide-cGMP pathway, and the combination doesn't produce a modest additive effect — it produces profound, sometimes refractory hypotension, which is why the interaction is treated as an absolute contraindication rather than a dosing caution. Tadalafil's own half-life, roughly seventeen hours, is why its labeled nitrate washout is forty-eight hours rather than the twenty-four used for sildenafil; taken daily, there is no clean window to wait out, because a new dose is due before the last one has cleared. The question the team is actually facing isn't just which second agent works — it's whether a six-year-old prescription for a completely different problem has just taken the guideline's usual answer off the table, and what to do about that.
In clinic, weighing the next agent
Start ranolazine, not isosorbide mononitrate, and I don't think this is a close call once the pharmacokinetics are laid out. He takes tadalafil five milligrams every day, not as needed before he expects to need it — every day, indefinitely. That matters because tadalafil's half-life is roughly seventeen hours, more than four times sildenafil's, which is exactly why its labeled nitrate washout is forty-eight hours rather than the twenty-four we'd use for sildenafil. A washout window describes the time after a single dose clears. He has no single dose to wait out — he's at steady state, and stopping today only starts a clock that doesn't finish for two more days, repeated every time the next dose comes due. Long-acting nitrates and PDE5 inhibitors both converge on nitric oxide-cGMP signaling in vascular smooth muscle, and the combination isn't mildly additive, it's synergistic — profound, sometimes refractory hypotension is the documented outcome, which is why this is labeled an absolute contraindication and not a caution.
Ranolazine isn't a free pass just because it avoids that specific mechanism — it's a CYP3A4 substrate and a moderate CYP2D6 inhibitor, which means it can raise his metoprolol levels somewhat, and it has its own dose-dependent QT effect through IKr block, worth real attention given where his baseline QTc already sits. That's a different, considerably more manageable category of risk than what nitrates would hand us here.
I'm not arguing with the mechanism — I'm arguing with how fast we're closing the door on the better-studied option. Long-acting nitrates have decades of trial and clinical experience behind them for symptom control. Ranolazine's evidence base, even now that current guidance treats it as a parallel add-on rather than something held in reserve behind nitrates, is still thinner where it counts. The largest outcomes trial we actually have, MERLIN-TIMI 36, enrolled patients with acute coronary syndrome, not stable outpatients like him, and even in that higher-risk population it didn't significantly reduce the composite of cardiovascular death, MI, or recurrent ischemia. It did significantly cut recurrent ischemia and worsening angina, which is a real and relevant finding for a man in his situation, but it's a thinner symptom-endpoint base than the one nitrates have accumulated. Neither drug earns him a mortality benefit — ISIS-4 and GISSI-3 both came back neutral for chronic nitrate therapy, so nitrates are a symptom drug too, and I want to be clear I'm arguing about symptom evidence, not prognosis.
So before we write nitrates off permanently, I'd want one honest conversation with his urologist about whether the daily dosing has to stay daily. If he and his urologist are willing to consider an as-needed regimen instead, a real forty-eight-hour window becomes achievable, and we're not choosing between two therapies on unequal evidentiary footing.
I've had a version of that conversation with him before, about a different medication, and I'd be cautious about reopening it today. He's been on daily tadalafil for six years, for both the BPH symptoms and his erectile function, and he's told me directly, more than once, how much the daily, no-planning-required part of it matters to him and to his marriage. Asking him to trade that for a drug class he's never taken, on the same visit where he's absorbing a new diagnosis-adjacent complication, risks a conversation he agrees to in the room and quietly doesn't follow through on at home. That's the ordinary way adherence breaks down when a change feels imposed rather than chosen, and I don't think today's visit is the moment to test that with him.
Start ranolazine today, with a baseline ECG and a repeat at steady state given where his QTc already sits, and let the tadalafil conversation happen on his terms, with his urologist, if and when he's ready to have it — not as a condition of getting his angina controlled. If ranolazine doesn't get him where he needs to be, that changes the calculus, and I'd expect all of us to say so plainly. But that's a different visit than this one.
Agreed: ranolazine 500 mg twice daily starting today, with a baseline ECG obtained before the first dose and a repeat ECG planned at steady state to check the QTc delta. Metoprolol succinate and amlodipine continue unchanged. Tadalafil continues unchanged; no urgent conversation about it today. Follow-up in four to six weeks to assess anginal frequency and decide whether ranolazine is doing the job at this dose.
