If she were six months post-infusion with a normal CD19+ count, I wouldn't be raising any of this — the guidance exists for exactly the situation she's not yet in.

Clinical Immunologist Response

I'd push back gently on treating the six-month figure as the actual safety threshold rather than the proxy it was built to be. The IDSA immunocompromised-host guideline's own rationale for the anti-CD20 interval is B-cell reconstitution — the CDC's own vaccination guidance for altered immunocompetence states this explicitly, recommending live vaccines only once both B-cell and immunoglobulin levels have normalized, not simply once a calendar interval has elapsed. Most patients don't have that number drawn on a schedule that lets anyone check it directly, so the guideline defaults to a population interval. Yolanda's flow cytometry already answers the actual question the six-month rule is standing in for, and it answers it the same way the calendar does: she isn't there.

So I'm not disagreeing with the rheumatologist's read of her safety — I'm making the case that we should be using her real number to decide this, not the interval, because the interval and the number point the same direction today, but they won't necessarily agree six weeks from now, and that's the actual decision in front of us.

Even setting the depletion question aside for a moment, there's a second clock running that neither of you has priced in yet. Rituximab's own prescribing information puts B-cell recovery as beginning at approximately six months after a completed course, with median B-cell levels returning to normal by twelve months — and it also records that a small minority stay depleted far longer than that. Even in the best case where her count turned around faster than average, a 17D vaccine given today or next week still needs roughly ten days to produce a detectable neutralizing antibody response in a healthy host, and hers isn't healthy in the relevant sense. A live vaccine given into an inadequately reconstituted immune system risks not protecting her at all even if it doesn't make her sick — which would leave her holding a certificate that satisfies the border requirement without the immunity it's supposed to certify.
