A TP53 Co-Mutation, an Excellent Scan, and a Trial With a History

A single patient responding well to osimertinib, whose co-occurring TP53 mutation predicts an earlier-than-average resistance. The disagreement is whether her own baseline lung vulnerability makes her exactly the wrong candidate for a combination strategy meant to delay that resistance.

Presentation

M.O., 55, has taught high school chemistry for nearly three decades and has managed rheumatoid arthritis on hydroxychloroquine for eleven years without much drama — well-controlled joints, the occasional flare around grading season, and a baseline chest CT four years ago that showed mild, asymptomatic reticular changes at both lung bases, read at the time as early RA-associated interstitial change too subtle to treat on its own. Three months ago, a persistent dry cough she'd blamed on chalk dust led to a new diagnosis: stage IV lung adenocarcinoma with an EGFR exon 19 deletion, and, on the same sequencing panel, a co-occurring TP53 mutation. She started osimertinib and has had an excellent early response — her primary tumor has shrunk by more than half on her first surveillance scan, and the cough is gone.

The TP53 finding is not simply a second mutation sitting alongside the one that matters: EGFR-mutant tumors that also carry TP53 alterations have repeatedly shown shorter time to osimertinib resistance in biomarker analyses of the drug's pivotal trials, meaning her excellent scan today isn't, by itself, evidence she's on a long trajectory. Her academic center is currently enrolling a trial pairing osimertinib with a checkpoint inhibitor specifically for patients like her, aimed at delaying that expected resistance — the same general strategy that, in a different trial years earlier, produced pneumonitis severe enough to close a treatment arm before it finished. She is, independently of any trial, already a patient whose lungs have shown they can develop fibrotic change under provocation, and her rheumatologist has noted her RA has stayed quiet enough on hydroxychloroquine alone that no one has ever needed to reach for a more aggressive immunosuppressive regimen — a baseline stability that a combination-therapy toxicity, if it occurred, would put at real risk of unraveling alongside her lungs. Whether that history makes her precisely the wrong candidate for added immunologic lung risk, or simply someone who needs closer watching either way, isn't a question the TP53 result alone can answer.

I'd enroll her in the combination trial now, while she's responding well. TP53 co-mutation has repeatedly predicted shorter osimertinib progression-free survival in biomarker analyses of FLAURA and related cohorts — her excellent early scan doesn't change that molecular reality. Acting now, before resistance emerges, is the whole point of a resistance-delaying strategy.

Pulmonologist Response

TATTON's osimertinib-durvalumab arm was stopped after interstitial lung disease appeared in 13 of 34 patients — 38%, against roughly 2 to 3% for either drug alone, with 5 of those events at grade 3 or 4. And read that arm's entry criteria: it excluded anyone with a history of interstitial lung disease outright. She has bibasilar reticular change on a CT already in her chart. The trial that produced this signal wouldn't have enrolled her, and the trial she's being offered is testing the same pairing.

You're right that the TP53 finding is a real signal, not noise — but a signal about a future event isn't the same as evidence that pre-empting it with combination therapy actually improves her survival, and the one time this exact combination was tested at scale, it hurt patients' lungs badly enough to stop the trial.

There's a middle path that doesn't require resolving whose risk estimate is right. Continue osimertinib monotherapy, but shorten her surveillance interval and add serial ctDNA testing — the principle APPLE tested directly — it used ctDNA re-emergence, rather than radiographic progression, as the trigger to change therapy in EGFR-mutant disease, though it did so from a first-generation TKI rather than from osimertinib, so we'd be borrowing the trigger, not the sequence. That preserves the option to escalate the moment there's real molecular evidence resistance is starting, rather than committing to a documented lung-toxicity risk against a resistance event that hasn't happened yet.

The oncologist wants to act early given the TP53 finding; the pulmonologist wants radiographic correlation first, consistent with the same lung-risk caution that shaped today's decision.
