Biologic Monotherapy Versus a TNF Inhibitor and Methotrexate Combination When Methotrexate Isn't Tolerated
Neither csDMARD she's tried has been tolerable, which forces a choice no trial has directly tested: a biologic proven superior to a TNF inhibitor alone, against a TNF inhibitor's own proven advantage only when paired with methotrexate.
Priya R., a 45-year-old woman, opened her catering business twelve years ago out of a single rented kitchen and just signed the lease on a second one, a milestone she's been too busy expanding into to fully enjoy. Her rheumatoid arthritis is two years old, and neither of the two conventional synthetic DMARDs tried so far have been tolerable: oral methotrexate brought on nausea severe enough to affect her appetite for days after each dose, and switching to subcutaneous methotrexate didn't fix it — her ALT climbed to 2.5 times the upper limit of normal within six weeks, and the drug was stopped. Leflunomide, tried next, produced diarrhea she describes as impossible to work through a twelve-hour catering shift with, and was also discontinued after five weeks.
She now needs a biologic that can stand on its own, without a conventional synthetic DMARD behind it, and the evidence for doing that well is thinner than it first appears. ADACTA, the only trial that has directly compared two biologics as monotherapy against each other, found tocilizumab outperformed adalimumab when neither drug had methotrexate riding alongside it — a real, randomized result, and the closest thing available to her actual situation. What it doesn't answer is whether tocilizumab alone reaches the level of control a TNF inhibitor achieves once methotrexate is added, since no trial has run that specific comparison; ORAL Strategy, testing a different drug entirely, found that even a genuinely effective JAK inhibitor given alone fell short, numerically, of what the same drug achieved combined with methotrexate. Whether that same gap applies to tocilizumab is an inference, not a result anyone has actually shown. And underneath both questions sits leflunomide's own chemistry: its active metabolite recirculates through the gut and liver and can linger in the body for close to two years without a deliberate elimination procedure, which matters directly if the next drug asks the same liver to prove it's clear.
Choosing a biologic with no csDMARD behind it
I'd start tocilizumab as monotherapy. ADACTA is the one trial that actually tests this exact situation — a biologic with no methotrexate alongside it — and tocilizumab beat adalimumab head-to-head under those conditions. That's the comparison she needs, not a comparison that assumes methotrexate is available.
ADACTA is real and I'd start there too, but I want to be precise about what it shows. It tells us tocilizumab alone beats adalimumab alone. It does not tell us tocilizumab alone matches a TNF inhibitor combined with methotrexate, which is the standard she'd be getting if methotrexate were tolerable.
The reason that gap isn't just theoretical: ORAL Strategy, testing tofacitinib, found monotherapy fell short of the combination arms numerically even though tofacitinib is itself a genuinely effective drug alone. If a comparably strong monotherapy option can still underperform combination therapy, assuming tocilizumab monotherapy closes that same gap is an extrapolation, not something ADACTA actually tested.
I'm not arguing against tocilizumab — I agree it's the right first choice. I'd just monitor her more closely than we would a combination-therapy patient, rather than treating ADACTA as proof this will land at the same level of control.
Before either drug starts, there's a mechanical step neither of you has raised: her leflunomide. Its active metabolite recirculates through the enterohepatic circulation and can still be measurably present nearly two years after the last dose without an accelerated elimination procedure — cholestyramine or activated charcoal, dosed to bring the level down faster. Starting tocilizumab, which carries its own transaminase-monitoring requirement, on top of an unwashed leflunomide exposure makes it genuinely harder to tell whose liver signal is whose if her enzymes move. I'd run the washout first.
Agreed: a cholestyramine washout course first, then tocilizumab monotherapy with transaminase monitoring on a shortened interval given the recent leflunomide exposure.
Not agreed: how much confidence to place in tocilizumab monotherapy reaching combination-therapy-level control. The rheumatologist's read is that ADACTA answers the question she actually has; the pharmacologist's read is that the real comparison remains untested, and the honest answer is closer monitoring rather than reassurance. Both plans converge on the same drug, for different reasons, left explicitly unresolved.
