Beta-Blockers in Heart Failure

Counterintuitive mechanism, three proven agents, and practical initiation rules

Why It Works: Acute Harm Becomes Chronic Benefit

Old Thinking

Sympathetic drive helps a weak heart pump

Reality

Chronic sympathetic activation injures the heart over time

At Start

Beta-blockers can transiently worsen pumping

Over Months

Ejection fraction improves with reverse remodeling

The Three Approved Agents

Non-Selective Plus Alpha-1

Carvedilol

  • Blocks beta-1, beta-2, and alpha-1
  • Vasodilating, more hypotension risk

Beta-1 Selective

Metoprolol Succinate

  • Extended-release, once daily
  • Not interchangeable with metoprolol tartrate

Most Beta-1 Selective

Bisoprolol

  • Once daily
  • Preferred with significant reactive airway disease
Only these three agents are proven in heart failure Do not stop abruptly during hospitalization
