Serotonin in Migraine — Triptans

Triptan mechanism, contraindications, ergot alkaloids, and migraine prophylaxis

Triptan Mechanism
5-HT1B Agonism
Meningeal Vasoconstriction
  • Meningeal vessels dilate during migraine
  • 5-HT1B receptors on vessel wall
  • Triptan agonism → vasoconstriction
  • Reverses vascular component of pain
5-HT1D Agonism
Trigeminal Neuropeptide Inhibition
  • 5-HT1D receptors on trigeminal terminals
  • Inhibits CGRP release
  • Inhibits substance P release
  • Reduces neurogenic inflammation and pain signaling
Prototype and Use

Sumatriptan — prototype triptan; subcutaneous injection = fastest onset (10–15 min); also oral and nasal formulations. Take at onset of headache phase for best effect. Not for daily prevention.

Contraindications
  • Coronary artery disease / angina — 5-HT1B receptors on coronary vessels; vasoconstriction risk
  • Uncontrolled hypertension — vasoconstriction worsens blood pressure
  • History of stroke or TIA — cerebrovascular vasoconstriction risk
  • Hemiplegic or basilar migraine — may worsen neurological deficits
  • Within 24 hours of ergotamine — additive vasospasm
  • Concurrent MAOI — reduced triptan clearance

Ergotamine / Dihydroergotamine
  • Non-selective: serotonin, dopamine, adrenergic agonism
  • DHE: intravenous for refractory migraine
  • Contraindicated: cardiovascular disease, pregnancy (uterotonic)
  • 24-hour rule: never combine with triptans
Prophylaxis
First-Line Preventive Agents
  • Beta-blockers (propranolol, metoprolol) — first-line
  • Amitriptyline — first-line; useful with comorbid depression
  • Valproate — first-line; teratogenic
  • Topiramate — first-line; cognitive adverse effects
  • Anti-CGRP monoclonal antibodies — newest class; monthly injection
