Presentation

Anja K., a 43-year-old woman, immigrated a decade ago and works as a translator, a career she says she chose partly because it lets her keep irregular hours around caring for her mother, who lives with her. She has chronic hepatitis B, well controlled on tenofovir for years, and was found to have superimposed chronic hepatitis D infection on routine screening prompted by persistently elevated ALT despite adequate HBV suppression — a pattern her hepatologist recognized as a real, if easily missed, signal of delta co-infection. Her most recent liver biopsy shows bridging fibrosis, more advanced than her HBV control alone would suggest, consistent with hepatitis D's typically more aggressive natural history.

Bulevirtide, the first therapy approved specifically for hepatitis D, is a genuinely new option for her — and the question in front of the team is whether to start it alone or combined with pegylated interferon. Bulevirtide monotherapy's pivotal evidence is MYR301 (Wedemeyer et al.), which ran 48 weeks against no treatment and reported combined virologic and biochemical response in a minority of patients; the combination with peginterferon has shown a stronger virologic response signal in patients whose disease stage most resembles hers, but interferon carries a real, well-documented side-effect burden — fatigue, mood changes, flu-like symptoms — that has historically driven meaningful early discontinuation in hepatitis therapy more broadly, a burden bulevirtide monotherapy largely avoids.

What the trial data assumes about tolerating interferon

I'd start bulevirtide alone. It's meaningfully better tolerated than interferon-based regimens, and interferon's side-effect burden — fatigue, mood changes, flu-like symptoms — has historically driven real early discontinuation in hepatitis therapy generally. Starting with the option she's more likely to actually complete seems like the reasonable first move, even if it isn't the option with the single strongest efficacy signal on paper.

I'd go with combination therapy from the start instead. The virologic response signal is meaningfully stronger for combination in patients at her disease stage specifically, and her bridging fibrosis means she has less room for a slower or partial response than an earlier-stage patient might. Given how advanced her fibrosis already is, I don't think tolerability should outweigh a real, stage-relevant efficacy advantage.

'Better tolerated' is true on average, but her fibrosis stage is exactly the situation where the combination's stronger response matters most, which is a real, not secondary, consideration against defaulting to the gentler option.

Clinical Pharmacologist Final

I don't think this should be decided on trial averages for either position without a direct conversation with Anja first. She works full-time and cares for her mother at home, and interferon's real-world discontinuation rate has historically been driven by exactly the kind of burden — fatigue, mood changes — that could be genuinely harder for her to absorb than for the trial population that established combination's efficacy edge. Before choosing combination on the strength of stage-relevant data alone, I'd want her own honest assessment of what she can actually sustain, since a regimen she can't complete doesn't deliver the trial's benefit regardless of what the data shows on paper.
