Vincent A., a 62-year-old man, lives alone on a small property outside town, where he spends most weekends restoring a decades-old sailboat he jokes will probably outlast him; a long stint as a merchant marine engineer explains both the property's private dock and his complete patience for the boat's slow pace. He has cirrhosis from longstanding hepatitis B, well-suppressed on tenofovir, and was found to have unresectable hepatocellular carcinoma on surveillance imaging six weeks ago — multifocal disease not amenable to resection or transplant, appropriately staged for first-line systemic therapy. Three months before that diagnosis, he had a variceal bleed requiring endoscopic banding, his first, treated successfully at the time.
The combination of atezolizumab and bevacizumab is the most established first-line systemic regimen for unresectable HCC, on the strength of IMbrave150 (Finn et al.) — a trial that required screening endoscopy and treatment of varices before enrollment, and excluded untreated or incompletely treated varices with bleeding risk. Bevacizumab's antiangiogenic mechanism carries a real, described bleeding risk, and its own prescribing information flags recent significant bleeding as a specific caution. The dual checkpoint regimen of tremelimumab and durvalumab, established as first-line by HIMALAYA (Abou-Alfa et al.) and carrying no antiangiogenic component, offers an alternative without that specific flagged risk — a genuinely relevant alternative for a patient whose recent history lines up almost exactly with the caution the more established regimen carries.
Whether the flagged risk still applies today
I'd choose the dual checkpoint-inhibitor regimen over atezolizumab-bevacizumab. Bevacizumab's own prescribing information specifically flags recent significant bleeding as a caution, and a variceal bleed three months ago is about as direct a match to that flag as a patient's history can present. The non-bevacizumab-containing alternative avoids that specific, described risk entirely without giving up a viable first-line option.
Before we rule out the bevacizumab-containing regimen on that basis, I'd want to know whether his varices have actually been adequately treated since the bleed. He hasn't had a repeat endoscopy since the banding episode, and 'bled three months ago' and 'currently has well-controlled varices confirmed on recent surveillance' carry meaningfully different bleeding-risk implications for a regimen decision like this. We're reasoning from a three-month-old data point when a more current one might change the picture.
I understand wanting more current data, but I'd be cautious about treating an unconfirmed 'probably fine now' as equivalent to a confirmed current variceal status — the flagged risk in the label is real regardless of how reasonable it is to expect improvement since the bleed.
Whichever regimen this ends up being, I'd want his overall liver function and portal hypertension control confirmed as adequate for either option before we finalize anything — his Child-Pugh A status is reassuring, but that's a snapshot, not confirmation that his portal hypertension specifically is otherwise well controlled. I'd support getting the repeat endoscopy Hepatology is asking for; if it confirms well-controlled varices, that's genuinely useful information regardless of which regimen we end up choosing, and it doesn't cost him meaningful delay to get it before starting therapy.
