Benzodiazepine Taper in Long-Term Prescribed Dependence: Direct vs. Cross-Taper
After eleven years on alprazolam prescribed for a real, understandable reason, a patient agrees to taper — and finds there’s no single agreed way to actually do it.
C.M.'s new primary care physician was the first person in eleven years to raise her alprazolam directly, rather than simply renewing it — flagging real, accumulating concerns specific to her age: increased fall risk, and a documented association between long-term benzodiazepine use and cognitive changes in older adults. She is sixty-eight, has taken alprazolam 0.5 mg three times daily for anxiety since her husband's cancer diagnosis eleven years ago — the illness he survived, and that she brings up early in most visits — prescribed originally by an oncology-adjacent primary care physician who has since retired. She has no history of illicit substance use, no history of dose escalation beyond her original prescription, and describes the medication candidly as something she now takes “out of habit as much as need,” though she is genuinely afraid of what stopping might feel like after this many years on it. She has never been hospitalized for any reason related to the medication and takes no other regular prescriptions besides a daily multivitamin.
She agreed in principle to tapering, rather than waiting for a fall or a memory complaint to force the conversation later, but was startled to learn there isn't one universally agreed method. She asked plainly whether simply taking a little less each week would work, or whether something more involved was actually necessary given how long she's been on it — she had assumed, reasonably, that there would be a single standard protocol everyone followed by now.
Her husband, seated beside her, offered that he'd support whatever plan she and the doctor thought was safest — the same steady presence he's been since the diagnosis that first put her on the medication, the two of them still out most evenings now for a slow walk around the neighborhood, weather permitting.
Eleven years on a short-acting drug, and no single agreed way off it
I'd taper her directly on alprazolam itself, in small, gradual increments over an extended timeline. She has no history of dose escalation, no prior taper attempts to learn from, and introducing a new medication — even temporarily — adds its own adjustment period and complexity for a patient who's already anxious about this process. Many patients do successfully taper off short-acting benzodiazepines this way with a sufficiently slow schedule.
I'd lean toward cross-tapering her to diazepam first, then reducing from there. Alprazolam's short half-life produces sharper interdose fluctuations — her blood level dropping and rising more abruptly between doses — which can make withdrawal symptoms more pronounced and harder to predict than diazepam's longer, more self-tapering profile. This is a well-established approach, described at length in the Ashton Manual, one of the most widely used clinical references for benzodiazepine withdrawal specifically because of this pharmacokinetic difference.
That reasoning is sound in general, but she has no dose-escalation history and no prior withdrawal difficulty on record — assuming she'll have a hard interdose course before we've actually observed anything may be solving a problem she doesn't necessarily have.
I don't think we need to commit fully to either approach before we've actually watched how she does. Start with a cautious, small first reduction on her current alprazolam, and if she experiences clear interdose symptoms — anxiety or physical symptoms clustering predictably before her next dose is due — that's real information pointing toward cross-tapering to diazepam. If she tolerates it well, we continue the direct approach. Either way, we move slowly given her age and this being her first attempt.
Agreed: begin with a small, closely monitored direct reduction on her current alprazolam, with an explicit plan to cross-taper to diazepam if interdose symptoms emerge, rather than committing fully to either method in advance.
Not agreed: whether every patient in her situation — long-term, stable, no escalation history — should start with a direct taper by default, or whether cross-tapering to a long-acting agent should be the standing first approach given the pharmacokinetic reasoning behind it. The clinical pharmacologist would default new patients toward cross-tapering from the start; the primary care physician prefers direct tapering as the default, reserving cross-taper for patients who show they need it.
